Preliminary information indicates that a 3rd party individual, not performing work on behalf of PG&E, was inside the manhole and made contact with PG&E equipment resulting in electrocution. |

On August 9, 2024, at 0131 hours, PG&E's Distribution Operator received notification that Circuit Breaker 2100/2 locked out de-energizing 21kV auxiliary bus and bank 4 at the Milpitas Substation in the vicinity of S Milpitas Blvd and Pepper Place in Milpitas, Santa Clara County, a non-HFTD. The responding electrician found no hazards, cleared targets and re-energized. Bank 4 carries no load and so there was no outage, and no customers were impacted. Before the electrician left the substation, Milpitas Police came to the Substation gate and advised electrician of a burn victim outside the substation wearing only one shoe. The electrician went back out into the substation and discovered the matching shoe and observed a ladder from the substation leaning against PG&E equipment. The victim was transported via ambulance to a local hospital and PG&E has confirmed they have been admitted for hospitalization. |

The responding troubleshooter learned that a third-party individual, not performing work on behalf of PG&E, was operating a scissor lift and contacted made contact with the overhead primary lines. The individual was transported via ambulance to a local hospital, and PG&E confirmed that the individual is being admitted for hospitalization. |

Power was restored to most customers by March 29, 2024, with final repairs completed on April 3, 2024. |

It was reported by major media that a tow truck driver fell asleep while driving and crashed the vehicle he was driving through a fence and into a power pole. Further it was reported that while a member of the public tried to assist the tow truck driver, he was shocked and transported to UCSD Medical Center for treatment. The injured man was said to be responsive and talking to emergency responders. While the shock does not appear to be attributable to utility facilities, the injuries did coincide with a vehicle collision into utility facilities, and thus SDG&E is reporting out of an abundance of caution. |

Three third-party individuals were attempting to retrieve their drone that got stuck in a tree. One of the individuals used an aluminum ladder to climb into the tree and brought a metal pole to attempt to free the drone. The individual who climbed the tree struck a nearby overhead conductor with the pole and received a fatal electric shock. On October 5, 2023, PG&E responded to an outage in the vicinity of E McKinley Ave just west of N Maple Ave in Fresno, a non-HFTD. The area is served by the Barton 1106 12kV overhead distribution circuit. The troubleshooter was searching for the cause of the outage when nearby unhoused individuals approached and explained that another unhoused person attempted to cut into a 600A riser, resulting in an explosion and was thrown away from the pole. The Fresno Fire Department responded, and the injured party was taken and admitted to Community Region Medical Center (CRMC) in Fresno. |

A vehicle collision where the vehicle exited the roadway and struck a transformer on the sidewalk in front of 1350 Euclid Ave in San Diego. The police believe that after the initial collision, the passenger exited the vehicle and tried to dislodge the vehicle off of the transformer. As the passenger was attempting to dislodge the vehicle she was electrocuted and died at the scene. This caused a fire to erupt, and the driver exited the vehicle and was also electrocuted (she was transported from the scene). While the fatality or hospitalization does not appear to be attributable to utility facilities, the injuries did coincide with a vehicle collision into utility facilities, and thus SDG&E is reporting out of an abundance of caution. |

SDG&E reported the incident on August 3, 2023, meeting significant media coverage criteria. |

On October 18, 2022, at 2101 hours the San Francisco Larkin (Y) 1108 12kV underground primary distribution circuit experienced a sustained outage affecting 2069 customers. PG&E troubleman responded to the area and discovered a failed underground distribution transformer bank near Ellis Street and Gough Street in San Francisco (a non-HFTD). Troubleman also discovered a splice failure near Laguna Street and Cleary Court. It is unknown if the outage was initially caused by the failed transformer bank or the splice failure. At 2255 hours 1602 customers were restored, leaving 467 customers still without power. PG&E received four reputable media inquiries into this incident within six hours of the incident. |

Preliminary information indicates SCE field personnel responded to the subject address and found a secondary service cable corroded at Structure No. P5178810 which serves the subject location. According to the LACFD report, the fire started within the walls of Unit 32 and the cause of the fire was listed as unknown and unintentional. There was no extension of the fire into other units. However, due to smoke movement the firefighters created inspection holes within the adjacent Unit 33. |

On September 5, 2022, at 1400 hours, Circuit Breaker 2105/2 opened at the Peabody Substation. At 1405 hours, PG&E received SmartMeter signals indicating a power outage impacting 5,900 customers. At 1405 hours, the City of Vacaville Fire Department ("VFD") responded to a fire alarm at Leisure Town Road and Vanden Road ("Incident Location") arriving at 1415 hours. At 1410 hours, PG&E Emergency Dispatch received a call from the VFD to report a power pole and brush fire and requested PG&E's assistance at the Incident Location, a non-HFTD. According to the VFD's report, the vegetation fire held at 20'x100' and was under control by 1435 hours in the vicinity of a railroad track ("RR"). The last fire unit departed the Incident Location at 1500 hours. The Incident Location is served by the Peabody 2105 21kV Overhead Distribution Circuit. |

On August 5, 2022, a PG&E troubleman responded to a fire near Running Deer Road and Gage Irving Road in San Luis Obispo County. At the fire location, the PG&E troubleman observed that a tree had fallen into PG&E conductors on the Templeton 2110 21kV Distribution Circuit and was on fire. Firefighters extinguished the fire at approximately 1/2 acres. The incident resulted in an outage to 1,325 customers. A CAL FIRE investigator on site gave the PG&E troubleman a form that alleged that the fire was a result of the downed powerlines. On June 22, 2022, at 0708 hours, the Kerckhoff-Clovis-Sanger #1 115kV transmission circuit relayed, resulting in a momentary outage affecting 7,263 customers. PG&E completed a full line patrol of the Kerckhoff-Clovis-Sanger #1, 115kV transmission circuit. During the patrol, we discovered that fourteen (not all contiguous) wood transmission poles had failed (broken, shattered, or splintered) with wires down in the area of Highway 168 east of East Shepherd Avenue in Clovis, a Non-HFTD. At the time, there was an ongoing abnormally high wind event with lightning strikes in the area. We believe the transmission poles failed due to the abnormally high winds with estimated wind gusts over 60 mph. In the vicinity, there was a vegetation fire that self-extinguished within approximately 5 acres. PG&E replaced all fourteen poles within 24 hours. The property damages to PG&E equipment resulting from this incident exceeded $50,000. |
