1. In California, have you had any issues with your phone sending false disconnected/out of service messages so that callers received such a message when your phone was in fact in service, or should have been in service?  
           
If you answered yes to Question #1, please provide details about when and where this happened, your phone number, and any information about the calling parties.    
2. In California, have you heard a phone system message saying "the number you have dialed is disconnected or is no longer in service" for phone numbers you believe are in service?  
           
If you answered yes to Question #2, please provide details about when and where this happened, the telephone number you called that gave you this message, and the phone number from which you were calling, as well as your contact information.    
3. In California, have you had any issues with your phone service not accepting collect calls, though you authorize the collect call?  
           
If you answered yes to Question #3, please provide details about when and where this happened, your phone number, and any information about the calling parties.    
4. In California, have you been unable to place a collect call to a phone number?  
           
If you answered yes to Question #4, what was the number you tried to call, when did you try to call and the phone number from which you were calling.    
5. In California, have you seen conditions with telephone facilities that you believe pose a danger to safety or reliability of communications service (e.g., low hanging wires, wires on the ground, leaning or bent poles, overloaded poles, one pole holding up another, frayed or broken cables, lines attached to trees?)  
           
If you answered yes to Question #5, when and where did this occur? If you took photographs please share them with us,and include your name and phone number so that we can contact you to let you know how to get the photos to us. Photos should include the name of the photographer, and the date and time it was taken, and the location of the facilities photographed.    
6. In California, have you been unable to reach 9-1-1 when you tried to dial 9-1-1 or during an emergency?  
           
If you answered yes to question #6, when and where did this occur? Please provide specifics including your name and phone number, the address from which you tried to call 9-1-1 and the type of phone service you used (wireline, VoIP wireline, or wireless).    
7. In California, have you experienced dial tone outages where you have not been able to make a call through a telephone service, or when you picked up the phone there was no dial tone so you were unable to place a call?  
           
If you answered yes to Question #7, when and where did this occur? Please provide specifics including your phone number and the type of phone service you used, wireline, VoIP wireline, or wireless.    
8. In California, have you been unable to reach 2-1-1 when you tried to call within a county that has enabled the 2-1-1 service; for example, have you received a “this number is disconnected or no longer in service” message even though you called from a county you know has 2-1-1? Have you been unable to reach 2-1-1 when calling from a business system that requires dialing 9 or another number to get an outside line? Have you had any similar problems calling 8-1-1 the “Call Before You Dig” number?
